Get started with Addo Forms

Learn what Addo Forms is, when to use it, and how the Forms workflow works.

Why use Addo Forms?

Addo Forms lets you create a reusable form that respondents can open, complete, and sign online. Instead of preparing and sending a new document to each person, you publish one form and share its public link.

You can use Forms for repeatable processes such as:

  • Vacation requests

  • Employee onboarding

  • Registrations and applications

  • Customer declarations

  • Requests for changes

  • Collecting information and documents

Note: This guide introduces the basic Forms workflow. It does not cover every component or advanced configuration option.

How Addo Forms works

A typical Forms process has five stages:

  1. You create a form in Addo.

  2. You add the information and questions respondents must complete.

  3. You preview and publish the form.

  4. You share its public link.

  5. Respondents complete and sign the form, and the result becomes available in Addo.

In this getting-started collection, we will build a simple Vacation request form. It asks for the employee's name, type of leave, requested dates, and any additional comments.

Open Forms Overview

  1. Sign in to Addo.

  2. Select Forms in the main navigation.

Forms Overview displays the forms available in your workspace. Depending on your permissions and configuration, the overview can show:

  • Status

  • Name

  • Group

  • Created date

  • Last edited date

  • Published date

  • Available actions

From here, you can create a form, open an existing form, preview it, and manage its availability.

Open Forms from the top menu, then select New to create a new form from the Forms Overview.

Ways to create a form

Select New and choose one of the following starting points:

  • Empty form creates a form that you design yourself.

  • Browse templates lets you start with a pre-made form and customize it.

  • Import form imports an existing form from a JSON file.

For your first form, this collection uses Empty form. This makes it easier to understand the form structure and the settings required before publication.

Choose how you want to create the form. To start with an empty form, enter a form name, select a group and language, and then select Create form.

Before you begin

Make sure that:

  • Your user has access to Forms.

  • You know which group should have access to the form.

  • You know which language respondents should see.

  • Your Addo subscription includes Forms publishing before you attempt to make the form public.
