Use Form Builder components to add information, questions, choices, calculations, and attachments to a form.
This guide explains the Standard component palette and the settings to review before publishing.
How to add a component
In Form Builder, select Standard, choose a component, and place it in a row.
Open the placed component to configure it. Most answer components include General, Validation, Style, and Dependent settings.
Content components
TEXT: Use Text for introductions, instructions, headings, and explanatory notes. Keep guidance close to the question it supports and use short, scannable paragraphs.
IMAGE: Use Image for a logo, diagram, or other supporting visual. Do not rely on an image alone for instructions a respondent must follow; include essential information as text too.
Input components
TEXT INPUT: Use Text input for a short, single-line answer such as a name, email address, reference number, or job title. Configure the component name, question, placeholder, data mapping, and tooltip as needed. Use Validation to make it mandatory when required.
TEXTAREA: Use Textarea for longer free-text answers, such as comments or a request description. State what the respondent should include and whether the field is optional.
NUMBER INPUT: Use Number input for quantities, amounts, or other values that need consistent numeric entry or will be used in a calculation. State the unit in the question, for example Amount in DKK.
DATE: Use Date for a calendar date. Use separate components for start and end dates, and review date restrictions or linked-date settings when the form needs a date range.
Selection components
Selection components use an Options tab.
Add the available choices before testing the form; a new Selection component has no usable choices until options are configured.
CHECKBOX GROUP: Use Checkbox group when respondents may select more than one option, for example services requested or documents attached. Use the Options tab to add each choice. Do not use it when only one answer is valid.
RADIO GROUP: Use Radio group when respondents must choose one option from a short list that should remain visible, such as Yes / No or a preferred contact method. Add choices under Options and use Validation if a response is required.
DROPDOWN: Use Dropdown when respondents must choose one option from a compact list, such as a department or leave type. A dropdown shows a placeholder until choices are added. Use Radio group instead when the list is short and comparing every option matters.
Advanced components
SEPERATOR: Use Separator to visually divide a long form into related parts. Use a Text component as well when the next part needs a clear heading.
CALCULATE: Use Calculate to derive a value from numeric inputs. In General, choose source components to build the formula and use Add, Subtract, Multiply, Divide, and bracket controls. You can set rounding, decimal places, and whether respondents see the formula. Test calculations with realistic values in Preview.
UPLOAD: Use Upload for supporting documents, receipts, or images. You can configure a placeholder, a maximum file size from 5 MB to 100 MB, allowed file types, and custom extensions or MIME types. Request only necessary files and make the field mandatory only when the attachment is required.

Confirm each component name and respondent-facing question or content.
Review every Selection option and required-field setting.
Test calculations with realistic values.
Check file size and allowed file types for uploads.
Use Preview to complete the form as a respondent.
